Cause of Death: Unknown — the critics’ darling of last winter, now released online — stands as one of the few truly compelling social dramas to emerge from Iranian cinema in recent years. Over the past four years, the country’s film industry has been dominated by two overwhelming currents: populist comedies designed to fill box offices, and what officials call “strategic productions.” The former enjoy full institutional support, from production permits to theatrical release, while the latter are often championed during production but rarely reach the public.

In this environment, what was once known as Iran’s “social cinema” has been pushed to the margins, replaced by underground productions crafted specifically for international film festivals. These films, conceived from the outset for foreign audiences, ride upon the social fault lines of Iranian society, exaggerating them to create a marketable product abroad — unburdened by domestic licensing or oversight.

It would not be an exaggeration to say that social drama in Iran never truly disappeared; it merely changed its course. Yet this shift has left behind films like Cause of Death: Unknown — works that pursue a genuine dialogue with society but suffer from neglect by both industry institutions and policymakers. Their absence from mainstream circulation reflects not only systemic disregard but also the cultural disorientation that defines contemporary Iranian cinema.

A look at the journey Cause of Death: Unknown has taken—from its very inception to its eventual release—reveals, in miniature, how deliberate state policies have driven Iran’s social cinema into a dead end. The film received its production permit in January 2021, moved into pre-production, and soon went into shooting. It applied to participate in the 40th Fajr Film Festival and even secured six out of seven votes from the festival’s selection committee. Yet, for reasons never made public, it was denied a screening license and excluded from the official lineup.

Afterward, the film was barred from domestic release, though it went on to screen at several international festivals and earned awards abroad. The familiar accusation of “blackwashing” — a label frequently used against works said to depict Iran in a negative light — appears to have served as the pretext for its exclusion from Fajr and its subsequent ban, even as no cultural official ever took responsibility for the decision.

Eventually, Cause of Death: Unknown was granted a screening permit in July 2023, though it wasn’t allowed a public release until December 2024 — a delay of nearly twenty months. Part of this prolonged limbo can be attributed to Iran’s distribution landscape, which remains largely inhospitable to non-comedic films. But that alone doesn’t explain the full picture.

A film withheld from Iranian audiences for four years — while celebrated at international festivals — inevitably prompted the question once it finally reached theaters: Why was this film banned? It was a question that no one within the country’s cinema establishment seemed willing, or perhaps able, to answer.

Ali Zarnegar’s debut feature — though his name was already well-known among Iranian cinephiles as the co-writer of Vahid Jalilvand’s Wednesday, May 9 and No Date, No Signature — unfolds as a road movie whose greatest strength lies, above all, in its screenplay.

The story follows a group of passengers whose journey takes a dark turn when one of them dies. A corpse and a large sum of money lie at the center of the drama, surrounded by characters each carrying their own private burdens. These interwoven micro-stories sustain the film’s tension and curiosity through to the end. Zarnegar and his cinematographer take the camera into the desert with a visual sensitivity rarely seen in recent Iranian productions, giving the film a striking texture and atmosphere.

Unlike many Iranian dramas, Cause of Death: Unknown never loses its rhythm in the second act. Though the narrative briefly drifts off course midway, it quickly regains its footing. By the conclusion, Zarnegar delivers a story told with clarity and drive, holding the attention of viewers attuned to social themes without resorting to blunt moralizing or overt symbolism.

Socially as well, the film stands apart. Its portrayal of how economic hardship deepens moral conflict feels far removed from the aesthetic and ideological clichés of Iran’s underground cinema. Over the past year, critics have frequently resurrected the old phrase “the noble social cinema” to describe Cause of Death: Unknown.

Ultimately, the film serves as a cautionary example of how Iranian filmmakers who work within the system — sincerely and in good faith — can still be punished for creating socially engaged works. In their absence, a different kind of cinema has flourished: one that exists entirely outside official frameworks and is never meant for public release.

Cause of Death: Unknown stands, in essence, as the antithesis of that underground cinema.
